Sending and Receiving Consent to Record Forms in Accuro - Interns/Residents

Sending Consent Forms

  1. Save the consent form to your computer from SharePoint. You should be able to locate it in the Intern & Resident Orientation folder.

  2. Open the form with Adobe Acrobat or Adobe Reader, whichever you have on your computer.

  3. Fill in the form with your client's information then sign the form with your digital signature.

  4. Open Accuro.

  5. Open HealthMail.

  6. Click the green plus to create a new message.

  7. Type in the recipient's name (last name, first name) then click on the appropriate person, enter a subject, and your message.

  8. Click the paperclip button and select From Computer.

     

  9. Find the consent form on your computer where you saved it in Step 1 – click the Look in drop-down to see your computer’s drives, folders, and files.

  10. Click Open once you find the file.

  11. You’ll know it’s attached when you see it beneath your message with a green checkmark next to it.

     

  12. Click SEND.

     

  13. Your client will then receive an email from Medeo. They'll download the consent form, sign with their digital signature, and attach it back to the message.

Receiving Consent Forms and Uploading them into the Client's Chart 

  1. Once they message you back, you will receive an email and their conversation will turn bright blue in HealthMail.

     

  2. Open the conversation and download the form they sent back to you by simply clicking the file.

  3. Once downloaded, click File to Chart.

     

  4. Fill in the appropriate information:
    Destination Folder: select Consent-Registration forms
    File Sub-type: Leave blank
    Description: type in Consent to Record Form

  5. Click OK.

  6. Check the Virtual Chart within their EMR to make sure the form was uploaded – it will be visible in All and Documents > Consent-Registration Forms.
