Projects or initiatives that should be created under the IST Portfolio must be submitted using the Project/Initiative Request form. To access the request form, sign in using your 8-character UWaterloo username @uwaterloo.ca (e.g. myuserna@uwaterloo.ca) and password. DUO authentication (2FA) is required to access and submit the form from off campus.
