Please have your department’s administrator submit a request for the new S4B extension. The request should be sent using this form: https://uwaterloo.atlassian.net/servicedesk/customer/portal/2/group/4121618/create/17033295 and include the following information:
the work order to which the monthly charge and any long-distance charges should be applied
username (your username is your 8-character UWaterloo username, i.e. j25rober@uwaterloo.ca)
current phone extension of the user (if none, please indicate)
location- building, room/cubicle number
