Collection of Access/Disclosure Requests for PHIPA Yearly Statistical Report

  • Only written requests under section 53(1) and 55(1) of PHIPA must be reported (including those that have been withdrawn or abandoned).

  • Count only written requests made by individuals, or their substitute decision-makers, for their own personal health information.

    • Section 26 has a ranking list of substitute-decision makers (power of attorney, lawyers, on behalf of the patient)

  • DO NOT COUNT third-party disclosure requests (i.e., WSIB or an insurance company for their own purposes/processing a claim, etc.)

Compiling Data for Report

From January 1, 2025 - May 6, 2025, you will be collecting the data from the Release of Information Tracking Spreadsheet.

From May 1, 2025 - onwards, you will be collecting the data through Accuro.

  1. Click Menu.

  2. Search and select Repliform CSV Export.

  3. Select the date range.

  4. Set the Form to Complex Access or Disclose Personal Health Information Form.

  5. Select Physician “Specialist, Health Information”.

  6. Click Export.
