What is it for?

If you are completing your program effective December 31, 2020, the UHIP policy allows you to extend your coverage so you can:

·stay in Canada to attend your convocation

·travel in Canada

I am graduating soon. How do I apply for an extension?

You must apply for your extension while your UHIP coverage is still in effect, i.e. by December 31, 2021

Once you have selected the number of months of extension, it cannot be changed.

The documents you require to remain in Canada must be in effect during the extension period.

What do I need to do to apply for an extension?

Make a bank payment to your student account using your student ID number as the account number for the appropriate amount indicated in the chart below

Fill out the attached UHIP application form

Scan and email your completed form and proof of bank payment as attachments to uhip@uwaterloo.ca.

The UHIP extension charge will then be added to your student account. Two to three weeks after you submit your extension application form, Sun Life will send an email to your uWaterloo email address which includes your UHIP coverage card as an attachment.
